3. Photo-bearing character certificate (original) from the Head of the institution last attended.
4. Three stamp-size (2 X 2.5 cm) coloured photographs
5. Parent’s/guardian’s income certificate (original).
6. *Original Caste Certificate.
7. Blood-group certificate.
8. Admit card of the admission test.
